Supplemental Group Term Life Insurance (SGTL)

Additional life insurance protection is available at group rates for employees, their spouse, and dependent children through Supplemental Group Term Life insurance (SGTL). This coverage applies regardless of the cause of death, including illness or accident. Rates are based on age of each covered individual. The available coverage options are outlined below:

Employees: An employee may purchase coverage up to six (6) times their salary. The salary is rounded up to the nearest $10,000, then used as the salary multiple. The maximum amount of coverage an employee may apply for is $1.5 million.

Spouse: An employee may purchase coverage for their spouse in $20,000 increments, up to a maximum of $260,000.

Children: An employee may choose on of three coverage options for dependent children up to age 26. Each option provides the selected coverage amount for each child and has a flat monthly premium, regardless of the number of children covered:

Important Notes


You may apply for or change supplemental life insurance (SGTL) at any time; it is not limited to the open enrollment period. When you enroll or request an increase in coverage, you must complete a health questionnaire for each individual. DMBA may contact you to schedule a nurse visit at your home or office to collect additional health information or samples. Coverage does not take effect until DMBA approves the application, a process that may take up to two months.

If a new employee applies for SGTL within 30 days of their hire date, $10,000 in coverage is available for the spouse and $5,000 for the dependent children without the need to meet health standards.

Changes in premium rates take effect every April 1st. For example, if your birthday is in June, you won't be charged your new premium rate until the following April.
