Forms We Do and Forms We Don't Do

FORMS WE HELP WITH:

  • We will need a Signed Release attached to the requesting email, including full name and birth month and day in order to complete this employment verification. Please be sure that it releases employment information and wage if desired.

    Please send the signed release to our email address.

    Please note: Be advised that sending full social security numbers is a violation of Utah State Code, as well as BYU policy. We request that in the future, you redact full social security numbers when sending documents via email. Our employees’ security is our top priority.

  • 630 DWS Information Form

    First, we will need a Signed Release in order to complete your request.

    You will need to fill out a 630 Employment information form for EVERY ACTIVE JOB you currently hold at BYU (Please make a small note at the bottom of each form to clarify which job the form refers to).

    Please fill out your name, case number, and customer signature at the bottom of the page. DO NOT PUT YOUR SOCIAL SECURITY NUMBER. We will fill in the rest and fax the form directly to the Department of Workforce Services for you. We will then send a scanned copy of the completed form to your email address.

    If easier, you may simply bring your blank DWS form to the HR Operations Center (2024 Wilkinson Center) in person and we will assist you with that required information here. Our office is open Monday through Friday, 8am to 5pm.

  • 631 DWS Information Form

    First, we will need a Signed Release in order to complete your request.

    Please fill out your name, case number, and customer signature at the bottom of the page. DO NOT PUT YOUR SOCIAL SECURITY NUMBER. We will fill in the rest and fax the form directly to the Department of Workforce Services for you. We will then send a scanned copy of the completed form to your email address.

    If easier, you may simply bring your blank DWS form to the Student Employment Office (2024 Wilkinson Center) in person and we will assist you with that required information here. Our office is open Monday through Friday, 8am to 5pm.

  • 116M Health Insurance Form

    First, we will need a Signed Release in order to complete your request.

    Please fill out your name, and birthdate, and erep case number at the top of the form. DO NOT PUT YOUR SOCIAL SECURITY NUMBER. We will fill in the rest and fax the form directly to the Department of Workforce Services for you. We will then send a scanned copy of the completed form to your email address.

  • First, we will need a Signed Release in order to complete your request.

    Make sure you sign and date your letter by hand. Then scan it (or take a picture with your cell phone) and send it to our email address.

    Please note: If you need this record for a specific reason (such as a green card interview, or job application), be sure to specify any additional information you would need included on this letterhead (such as average hours worked or specific wage information). If your employment pre-dates the year 2000, you are more likely to obtain the information you need by looking at paystubs, tax documents, or by contacting the department you worked with.

  • Please contact the third-party investigator and give them our contact information so that they can contact us directly with their request. Please do not try to obtain a verification on their behalf, because they will have their own requirements and forms that will need to be filled out regardless of any information you can provide.

  • Please fill out section 3 to the best of your knowledge, then either email us the form, or bring it in to us (2024 Wilkinson Center) in person and we will verify and sign the document on site. Our office is open Monday through Friday, 8am to 5pm.

  • Please send us an email with a detailed description of the information you need and why you need it (this can help us understand and format your verification according to the situation). Along with your request please include a signed release. To fill this requirement, you can simply write a letter stating what information you'd like us to release, to whom you'd like to release it, and include your full name and birth month and day. One example would be: “I authorize BYU to release any and all employment information including wage. My full name is ________. My birthday is ___month___ ___day___).” Please include either your BYU ID # or net ID.

    Make sure you sign and date your letter by hand. Then scan it (or take a picture with your cell phone) and send it to our email address.
