About
Don't leave yourself susceptible to the flu and sign up for your flu shot today!
The Flu Shot Clinic is open to students, faculty, staff, and their families. Because getting a flu shot is so important at this time of year to decrease the risk and severity of the flu, BYU Employee Wellness is partnering with the Student Health Center to offer flu shots to the BYU community. Please note that COVID-19 vaccines and boosters will not be offered at this clinic due to CDC guidelines. It is important to note that Medicare/Medicaid and the DMBA Deseret Alliance plan are NOT accepted insurances.
The clinic will be held September 22-24 from 7:30 am - 5:20 pm in the Garden Court of the Wilkinson Student Center.
Waiver and Insurance
You will need to fill out this year's waiver and then bring your insurance card (or $30) with you the day of the clinic. If you choose not to fill out the eForm, you will need to fill out a physical waiver at the clinic.
Medicare/Medicaid and Deseret Alliance are not authorized for this clinic. Accepted DMBA plans include DMBA PPO 90, DMBA PPO 70, DMBA HSA 80, DMBA HSA 60, and the BYU Student Health Insurance plan. Coverage for the flu shot by any other insurance provider must be confirmed by the patient.
Waivers are required of every individual who will receive a flu shot.
Please make sure you receive the confirmation email after making an appointment.
- The digital waiver will be sent in a reminder email to the same email address you received your confirmation email from 2 hours prior to your appointment.
