COBRA coverage is available to employees after their BYU termination date for up to 18 months. They must enroll within 60 days of being notified by DMBA.

Before the termination date, an employee can access the COBRA enrollment form on the DMBA website (login, MY HEALTH drop down menu, Forms Library, “COBRA Medical/Dental Coverage Enrollment Form”). After termination, a prior employee can contact DMBA at 1-800-777-3622 and get the enrollment form or further information.
